Privacy Statement and data Protection

I am registered with the ICO (Information Commissioners Office) which means that I need to tell you what data I collect, what I am doing with that date, how long I am keeping the data.

What data do I keep and why do I need it?

Name and Age- Basic information with the purpose of getting to know you

Address, Telephone number, email address- These details are used for contacting you and I will always check which method you prefer and if you are happy for messages to be left.

Doctors Details- If I was worried about you being a risk to yourself I may which to contact your doctor, this would not be done without your explicit consent.

Will I share your data and if I do who will I share it with and for what purpose?

It is very unlikely that I will share your data, I will NOT sell it or use it for any unethical reasons. I may share your notes for the following reason and this would be with your consent and prior knowledge;- if they were subpoenaed by court, if you request them, to my clinical supervisor in the event of my death, If you have advised me that you or somebody else is at risk. (In the event of a crime being committed with I am ethically and legally bound to report; your notes may be shared if requested and as per the regulations this will be without your consent.) You have the right to see, have amended or have destroyed any data that I may keep about you. I will respond to requests within 14 days.

How will I store your data?

Phone number and email address- These will be stored in my business phone and will be deleted 7 days after we have finished therapy. I have a record of these details on your intake assessment, this will be kept separately to your session notes, in a locked filling cabinet. When therapy ends any identifiable information will be destroyed, the rest of your assessment will be attached to your notes.

Session notes- your session notes will be a record of the session, they will be minimal and accurate, will not include your name, address or telephone number. The session notes will be stored in a locked filing cabinet if paper based or a secure password protected system if computer based.

How long will I store your data for and how will I dispose of it?

I will keep your session notes for 5 years from the end of therapy as required by insurance company. I will shred this privacy statement and your personal details from the intake form 7 days after therapy ends. I will delete your phone number and email address 7 days after therapy ends. Emails will be deleted when they are no longer needed, at the latest 7 days after therapy has finished.
